Out-of-Attendance Area Request Form

Administrative Policy No. 403 - Admission of Students
Parents requesting that their child(ren) transfer schools within Prairie Spirit School Division are asked to complete Section 1 and submit this form to the principal of the current school.
Section 1: Parent/Caregiver
Following a conversation, first with the principal of the current school and secondly, the school in which you would like to attend:
1. Principal of the current school will provide this form.
2. Submit the completed form to the principal of the current school.
3. After due process, you will receive a response to your request from the school superintendent.
